Camp participants must print the forms below, sign them, and send them to WSU at least ONE week prior to your visit. Please send signed forms to: Washington State University, PO Box 641062, Pullman WA 99164-1062, Attention: Syndi Jacobson. Faxes not accepted. Forms must be mailed and received before to the first day of camp.
